REQUIREMENTS
A minor guardianship of the estate may be dispensed with by the Court when a minor has received $25,000 or less, usually through insurance proceeds or inheritance.

NOTE:
- All paperwork must be typed or written neatly in blue ink.
- All filings must be single-sided. We will not accept double-sided originals.
- Please do not staple original paperwork. We cannot accept filings with staples.

- Photocopy of the Minor’s birth certificate
- Acknowledgment of Responsibility (FC Form 104.4-B)
- Consent to Application to Dispense with Appointment of Guardian for a Minor (FC Form 104.4-C)
- Household Resource Worksheet (FC Form 66.2-A) If applicable
- Proposed Judgment Entry Dispensing with the Appointment of a Guardian for a Minor (FC Form 104.4-D)
30 DAYS FOLLOWING JUDGMENT ENTRY
- Report of Distribution of Assets (FC Form 104.4-E)
- Verification of Receipt and Deposit (FC Form 22.3) If applicable
